Appointments/Immunization Schedule

• Patients are seen by appointment. Nursing staff will provide families assistance and help in assessing if a sick-visit appointment is needed. Every attempt will be made to see a sick child on the same day they are ill, if warranted.

• Follow-up visits and well-child visits should be scheduled well in advance of the time needed.

• The usual schedule for visits and/or immunizations is as follows:

Exam Immunizations

• Newborn Hepatitis B#1, Newborn Screen#1 (in hospital).
• 2 weeks- Newborn Screen #2
• 1 month- Hepatitis B#2
• 2 months- Pentacel (DTaP#1Hib#1IPV#1), Prevnar#1, +Rota #1
• 4 monthsPentacel ( DTaP#2Hib#2IPV#2),Prevnar#2, +Rota #2
 6 months-Pentacel ( DTaP#3Hib#3, IPV #3),Prevnar#3, +Rota #3
• 9 months– Hepatitis B#3, CBC (anemia screen).
• 12monthsMMR#1 ,Varivax#1Hepatitis A #1
• 15 months- DTaP#4Hib#4, Prevnar#4
• 18 months- Hepatitis A#2
• 2 years- Usually no immunizations/Catch up on missed immunizations
• 3 years- Usually no immunizations/Catch up on missed immunizations        
• 4 years- DTaP#5IPV#4MMR#2,Varivax #2
• 5 – 10 years-Check up every 1-2 yrs. Usually no immunizations.
• 11 years Menactra#1,Tdap, +Gardasil
• 13-18 years Check-up every 1-2 yrs. Usually no immunizations.
16-18 years-Menactra#2, (5 years from 1st dose)

•Flu vaccinations may be given in the Fall to children ages 6 months to 18 years of age.
